骨粗鬆症性椎体骨折のリスク要因としてのサルコペニアの現状および高齢者における上下肢筋肉分布について検討した。55歳以上の新規椎体骨折患者274例をVCF群とした。骨粗鬆症を疑いDXAを施行した外来患者2266例を対照群とした。VCF群で補正筋量(SMI)が有意に低値であった。対照群の31%に対し、VCF群の49%がサルコペニアに罹患し、VCF群で有意に罹患率が高かった。サルコペニアの診断、下肢SMIが有意な骨折のリスク因子であった。上肢SMIは有意な骨折のリスク因子ではなかった。その他には、サルコペニアモデルでは年齢、体重、全身骨BMDが椎体骨折に対する有意な因子であった。SMIモデルでは、男性であること、体重、全身骨BMDが有意な因子であった。

さまざまな研究施設で骨粗鬆症を疑い、全身DXAを施行した患者において骨密度と筋量の間に有意な相関を認めたと報告されている。廃用や低栄養、ビタミンD不足など、sarcopeniaと骨粗鬆症に共通した原因の関与が示唆される。Coinらも同様に、Sarcopenia患者において骨密度の低下をみとめると報告している。Sarcopeniaによる易転倒性と、骨粗鬆症による骨脆弱性の双方の治療が骨折予防に必要であろう。
世界的な人口の高齢化に伴い、高齢者の骨折も増えている。わが国で年間15万件発生するとされる大腿骨頚部骨折は、年間103万発生するとされる脊椎骨折とともに高齢者医療における最大の課題の一つである。骨折により、ADLは大きく下がり、高齢者寝たきりの主要な原因の一つである。加齢による筋肉量、筋力の減少であるSarcopeniaは、転倒の危険を高めることが知られている。大腿骨頚部骨折や脊椎圧迫骨折を代表とする高齢者骨粗鬆症性骨折は転倒などの軽微な外力で発生し、入院治療、看護、介護において大きく社会資源を消 費する。骨折の予防と治療は、逼迫した医療財政を救う鍵であり、そのためには骨折におけるsarcopeniaの影響を知る必要がある。
